In this video, we have presented a case of upward fixation of the patella in which the medial ligament of the patella on the stifle joint is luxated from its actual anatomical location and hinders the patella head while walking. The etiology of this disease has not been yet published.
The treatment of this case is simple although require a unique practice in locating the ligament properly. Afterwards incising the ligament is not a major issue. There are various approaches for this surgery. In this video Dr. Adnan maqsood has performed the vertical incision technique with a sharp blade.
The purpose of the video is to educate the farmers and field veterinarians that this problem is treatable and you can save expensive animals of the poor farmers and can seve the animal community by your slight effort. we hope to help any veterinarian willing to learn this procedure and help the suffering animals in pain.
In this video, we have presented a typical case of Upward Fixation Of Patella or commonly called String Halt which is scientifically not correct as Stringhalt is a condition occuring only in horses. This is a physical abnormality and proper etiology is unknown till today.
We have tried to show the simple method of location and Desmotomy (Cutting of Ligament) of Medial patellar ligament.
The anatomy of this surgery is critical like every other operation. Patella which is also called knee cap is surrounded by muscles, ligaments, and tendons. One of them is Patellar Ligament which has three heads
1. Lateral part (on the outside location of the hind leg)
2. Middle part (in between Lateral and Medial head)
3. Medial part (Inside location on the leg above the patella in the hind leg) shown in image
For this surgery Medial part is important, however, it is recommended to palpate all three parts for the confirmation. It is present initially in one leg called Unilateral String Halt or in both legs called Bilateral String Halt. Medicinal treatment has been tried and is usually unsuccessful or re-occurrence is very common. Therefore, Surgical solution is preferred and is very easy to be performed by a Licensed and Certified Veterinary Doctor only.
Once Confirmed, it is just incised after completion of all surgical protocols involved and detailed below.
1. Restraining of the animal in Standing or Lateral recumbency
2. Aseptic preparation of the site
3. Injecting Local anesthetic drug (Lignocaine/Lidocaine etc.) according to the principles
4. Locating the Medial Patellar Ligament (MPL) as described above and shown in image
5. Incising the straight part, moving the surgical blade in Medio-Lateral direction
6. Confirmation of the incised part
7. Flexing and Extensing the limb
8. Repeat same in the other legs in case of Bilateral occurrence
9. Slowly Rehabilitate the animal and help to stand or relax
