The following presentation shows the technique of trans-axilla frontal sinusotomy or the intact bulla technique. The role of wide exposure is emphasized to improve the access to the frontal recess with identification of the different anatomical landmarks especially the skull base and the anterior ethmoidal artery. This technique can be utilized in case of isolated chronic frontal sinusitis to achieve patent frontal sinusotomy without disturbing the anatomy of the anterior ethmoid.
