🧠 Microsurgical Resection of Insular Gliomas
✅ Gross total surgical resection of gliomas, including insular gliomas, is associated with improved progression-free and overall survival; however, the insular lobe is hidden by the frontal, parietal, and temporal opercula at the depth of Sylvian fissure, making resection of tumors of the insular lobe a significant challenge. This is particularly true on the dominant hemisphere, where injury to the frontal or parietal opercula can result in language deficits. Moreover, injury to branches of the middle cerebral artery during glioma resection can also result in deficits by causing infarcts of the internal capsule, basal nuclei, or corona radiata. For this reason, a detailed knowledge of the anatomy of the insular lobe and adjacent vasculature is required prior to attempting resection of insular gliomas.
