Osteochondroma

Pathogenesis

  1. Lump of cancellous bone covering cortical bone and a cartilaginous cap
  2. Originate from separation of small pieces of metaphyseal cartilage from the main cartilagnious epiphyseal plate
  3. Usually start to grow in adolescence

Approach

  • Expose relevant area
  • Examine as for lumps
  1. Inspect

    • Hemispherical lump
    • Solitary / multiple
  2. Palpate
    • Smooth surface
    • Narrow base
    • May be bony hard or soft if there is overlying bursa
    • Assess relationship with adjacent muscles and tendons

 

Treatment

  1. Excision if symptomatic (pressure, cosmesis)
  2. Suspicion of malignancy 

 

Diaphyseal aclasis

  1. AD - multiple osteochondromas particularly in the limb bones
  2. All bones that ossify in cartilage effected except spine and skull
  3. Thought to be a failure of growth-plate modelling
  4. Small incidence of sarcomatous change, particularly if subjected to trauma
